Insurance

Power of Movement PT is an in-network/participating provider with Emblem Health - GHI and HIP. We are an in-network exception provider with NYSHIP/The Empire Plan.

We are also an Insurance-friendly office that works with the family and their insurance company (as long as it is part of the insured’s plan benefit) to get services covered at the network level.  Insurance companies that PoM will work with are:

› Cigna

› Oxford

› United Healthcare

Power of Movement PT will work with you and your insurance company to limit the out-of-pocket expense incurred.

› Pediatrician’s or Referring Physician’s Prescription
› Completed PoM forms and Medical History Form (online form)
› Health Insurance Card
› Forms of payment: Cash or Check (we do not accept credit cards)
